Website Goal

The overall goal of this site is to elevate the quality of healthcare in Vermont by giving patients the tools to be more proactive and involved in their medical care.  This website is designed around the “medical home” model as a concept of care delivery that includes an ongoing relationship between a provider and patient - a comprehensive approach to care and coordination of care through multiple providers and community services.  In this medical home, the primary care physician helps patients get specialty care when they need it and, through the website’s records, keeps careful track of treatments and informs specialists of the patients’ progress.  We feel that by improving and enhancing the patient’s involvement in the collection and updating of their medical data, and their communication with their providers, that they will improve their overall wellness and insure they get the best care possible.

Connectivity

This site has the capability to be “attached” to a wide variety of other healthcare processes, such as your doctor’s Electronic Medical Record (EMR), your local hospital, Vermont’s Blueprint for Health, Visiting Nurse Services, as well as any wellness program. There are menu choices on the left that describe some of the many options that can be implemented to connect this patient portal with outside processes.

Provider to Provider to Patient Secure Messaging

Of great importance is the ability for providers to send messages between other participating providers and their patients. The system bypasses the normal emailing and is totally self-contained within the secure portal, complying with the strict HIPAA requirements. The connections between the professionals who work on each case are secure, seamless and convenient. Not only do doctors and patients have easy access to medical information, but, the patient, by tracking their daily vitals readings, provides the physicians with more comprehensive data. The model allows patients with chronic ailments to be contacted regularly to reinforce treatment regimens and see how they are doing.

Potential Uses

This portal has many health related uses. Here are a few examples:

  • It can integrate seamlessly into an existing EMR to enhance the communication between the patient and the physician, improving the patient’s involvement in their own health.

  • It can be used by colleges to assist students that have chronic diseases to better understand and maintain their existing conditions.

  • It can assist cardiac rehabilitation programs to better track their patients during and after completion of their programs.

  • Wellness programs for union members would benefit from tracking their medical data more closely by using this portal, and their controlling wellness center could be set to access their data and work with them to attain their healthcare goals.

  • A company could utilize this site to assist their workers with chronic diseases such as diabetes and cardiovascular diseases to better care for themselves.

  • The State of Vermont could sponsor new disease management programs statewide and have their participants track their progress using this portal.

How It Works For The Patient

My Vermont Medical Home is a secure website that allows individuals to become more involved in their medical care by helping them to track their specific healthcare concerns and conditions. Each person has their own set of records and has complete control of the privacy and outside use of those records at all times.

When you join the website, you will be asked a series of questions that defines who you are, what your current medical conditions and concerns are, and who your current doctors are. For each doctor, you will select what data you want them to be able to view or update. In this way, you get to control all your data and who can look at it!

If you give update permission to a doctor or provider, they will be able to enter their notes when they see you, and update lab and other test records pertaining to your care. These tests remain in your permanent record and can be reported to other doctors that have your permission to view the results. This saves countless hours and days of waiting for one provider to give information to another of your caregivers.
