Our customized patient portal provides an easy, web-based method for you, the patient, to directly update and report your medical information. The portal is designed to also provide study information and relevant news to you through the web-based system.

Once you have joined the website, you can begin to enter your medical information into your own secure portion of the website. Your information is only visible to providers that you identify, and expressly grant permission to. You also have the ability to select various parts of your data for access by your providers.  If you allow them, they can even update your information with additional medical data such as lab tests and other diagnostic tests, giving you a more complete picture of your medical information.

This section of the website provides a condensed guided tour of the various components of the portal.  To begin, here is a list of the sections within the portal, with a link to each for easy navigation through the tour. Just click on the section name you wish to read more about. To return to this section area, click the Back to Top button within each section.

Please click on the image thumbnails to see larger versions of each section.  Each image will open within its own pop-up window.  You will be able to close the pop-up windows and return to the main body of the guided tour.

General Patient Information

When you sign up for the website, you provide some general information to properly identify you to your doctors and healthcare providers. Here is the screen you would need to complete to store your unique data.

Insurance Information

For easy reference, we provide you with an area of the system to store all your insurance information such as the companies, the policy number and the group if there is one. This will optionally print out on your profile report to take to your provider visit.

Vital Readings

The vitals module will allow you enter daily readings for various measurements such as pulse, blood pressure, temperature, weight, and allows you to keep track of your levels and amount of exercise, and optionally to track your sugar levels and even your oxygen levels. The screen below shows how the system tracks your blood pressure and displays it over time on a graph for easy reference.

Lab Test Results

If you decide to give permission to your doctor's office to update your lab results automatically, you will have a historical record of all tests updated for reference.  You can also enter the results yourself, if you so decide.  All you need do is to ask the lab taking the tests to send you a copy of the results directly and you can enter them into this system.

Chronic Conditions

In order to properly identify your various medical conditions, we provide you with an easy "point and click" set of check boxes and "other" text boxes that you can use to list your various problem areas. You may also manually enter up to three chronic conditions that are not included in the default list.  Finally, you can enter your current smoking status, as smoking is a detriment to your health.  This information would then become a part of your overall medical profile described later.

Self-Management Goals

This module gives you the ability to set certain goals for achieving a healthier lifestyle.  Simply enter a target date and value for the goal that you wish to attain.  This module also includes the latest recorded labs and vitals values on file for each goal category, useful when determining your goal target value.

Medications

If you are taking any medications, we strongly recommend that you enter into the system each of those medications and the frequency.  In this way, when you print out your profile, you can have a quick list of the current medications to give to your doctor. Also, if you give them permission, they can check this information any time they log into your data.  It provides a quick reference to eliminate your forgetting to of a given medication.  It provides an area to enter your cost information as well for later reporting over time.

Supplements

As with medications, it is important to track your supplemental drugs.  Often times, there are potential interactions that your provider will be able to reference if you give them a complete list of all supplemental drugs you are taking.  The screen for entering these drugs is similar in format to the medication screen shown above.

Preventive Health Tests

There are many tests that your provider may ask you to take. This section details the date of your last test for a variety of the standard tests, such as cancer screening, dental exams, mammograms, and more.  This data will print out optionally on your profile and is available to your provider to reference online.

Allergies

An important area of track are your allergies to various environmental items as well as your drug allergies.  This portion of the system allows you to enter each in and stores them for later reporting.

Vaccinations

One area of data that is easily forgotten is the area of vaccinations. It is important to track the dates of each type of shot and to communicate this information with your providers.

Symptom Tracking

As you continue to keep your data up to date, you will need to recognize times when you have certain symptoms that are special, or that are worse than usual, especially in the area of pain management, loss of breath, an such.  This section allows you to enter in the various symptoms you are tracking and to assign a severity to each occurrence, noting whether the symptom is mild, moderate, or severe.  You can even enter whether the given symptom caused you to miss school or work.  This information is very important to communicate to your provider and appears on your profile.

Pain Management

Sometimes, you will experience certain aches and pains.  Some may be relatively minor while others can be debilitating.  This section allows you to enter in the various pains that you experience.  As with symptoms, you can assign a severity to each occurrence and can even specify whether the pain was severe enough to cause you to miss school or work.

Patient Notes

Periodically, you may feel the need to enter information that doesn't apply to vitals, lab tests, or other sections of the web site.  You can use the patient notes module to accomplish this.  You simply enter the date and the information that you wish to store.  Furthermore, you will be able to see notes that a provider has entered about you.

Surveys

Within this website is the ability for doctors to create a survey of questions that they would like sent to their patients.  When the patient completes the survey, the doctor as the ability to view the resultant answers from his patients.  The system tracks your answers and allows you to stop a survey and go back to it at another time. The screen below shows how a list of surveys can be available to you, as the patient, to select from, and the second screen shows a sample question and possible answers.

Doctor Maintenance

When you begin to use the patient portal, you are asked to identify doctors who may have access to your data.  Once identified, you can select the information groups that you want them to be able to view or print.  Furthermore, you will also be able to determine whether or not your data can be updated by your provider, or from an external source such as the HIE.  This allows you to selectively permit each doctor's office to view or update only the data you are comfortable sharing with them, hiding the remainder of the information you have stored.  At any time, you can modify these settings for a doctor to allow or disallow viewing of your information by section. The screen below shows how easy it is to modify your settings for a doctor.

Advance Directives

Within this system is a complete package of all the forms for completing the Vermont Advance Directives application. It is an eight page form that has all the questions you must answer to properly file your wishes.  As mentioned before, you can decide what doctors you want to have access to this information and to be able to view or print it out for their records.  The following two screens illustrate one of the forms to fill out and the associated instruction window for that form. All forms have a complete instructions for filling each page out.

Messaging System

In order to provide you with a method of contacting participating providers through this website, we have created a messaging system that allows you to select a doctor and create a message that will go through a secure network to their offices.  You can use this to notify them of the need for refills, for an office visit, or to ask a general question.  Similarly, we have given you an in box for all messages from your providers.  It tracks each message and preserves them in an archive to allow you to go back in time to refer to an old message.

Patient Profile

One of the most important capabilities of this system is the patient profile. This system allows you to select from a wide variety of your data to print to a report that you can then take to your provider or doctor's office.  You can even print an activity summary that contains historical graphs of your vitals over time.  The following screens show the selection options for printing the report each time, and the output reports for those selections.
