Our closed social network helps patients manage their chronic disease in a restricted, controlled environment.  It gives patients the tools to make better decisions and to take actions to improve their health – the very foundation of chronic disease management. It provides patients with informative articles, announcements, and other valuable resources about their chronic disease – providing knowledge that they can use to make changes in their lifestyle.  Patients will also have the ability to send email invitations to their friends and family, asking them to join them on our closed social network to be a part of the patient’s “fan club.”  

Providers also benefit from the closed social network. They have the ability to monitor the activity of the member patients that they’ve referred to see who has and who hasn’t joined the system within a certain period of time.  They can keep a finger on the pulse of the number of fans their patients are adding, and even see a demographic breakdown of their patients who are using the closed social network.

Our closed social network sits in a secure web environment and can be adapted for any chronic disease.  It also seamlessly integrates with our other web-based products, including our patient-centric medical home and our chronic disease registry applications. It can also optionally interface with an electronic medical record (EMR) system for automatic data updating of lab tests and vitals.

This section of the website provides a condensed guided tour of the various member, coordinator, and fan supporter functions of the closed social network application.  To begin with, here is a list of the sections within the closed social network, with a link to each for easy navigation through the tour. Just click on the section name you wish to read more about. To return to this section area, click the Back to Top button within each section.

Member - Home Page

When a member signs into the closed social network, they will see their customized home page.  It contains items that they can maintain, regional announcements and information, reminders, and a full reference center.

Member - Privacy Settings

A member has complete control over the information that they wish to share, including whom to share that information with.  This includes the member's referring physician, as well as the health care organization that physician belongs to.

Member - Texting Options

A member has the ability to subscribe to daily and weekly text messages, including visit and lab test reminders, as well as motivational messages sent by their regional coordinator.

Member - Supporter Settings

A member can also set permissions and settings for their supporters - the friends and family that they have invited to be part of their personal "fan club"..

Member - Statistics

The closed social network integrates seamlessly into our other web-based applications, including our chronic disease registry and our patient-centric medical home.  When a healthcare organization has either of these companion applications, the member's latest vital and lab statistics will be available to them for viewing.

Coordinator - Home Page

Coordinators are responsible for the general upkeep and maintenance of the closed social network application.  This includes maintenance of data files for their regional members, and posting of articles and other references for members in their region.  They can also enter articles and references that are available to all members of the social network, regardless of their region.

Coordinator - Content Management

Coordinators also have the ability to moderate content entered by members and their supporters, including challenges, "other" exercises, and supporter profile photos.  Inappropriate content can be removed from the system.

Supporter - Home

Friends and family who accept an invitation to join the closed social network will have the ability to set up their personal profile or interact with any members that they support.

Supporter - Member Functions

Supporters will have the ability to view certain information about that member, depending on the supporter access settings determined by the member.  They will also be able to communicate with the member via email, and issue physical activity challenges to the member.
