Button Systems has developed a "closed" social network component that can optionally integrate with our medical home software to enable the patient to develop a broader support system.  Only patients who use the medical home will be able to use the social network application - it is not available to the general public like Facebook or Twitter.   

Features:

• Seamless Data Flow - information can be shared between the medical home and social networks without having to enter the same information in both systems.  Information entered in one system automatically populates data displays within the other.  For example, a date that is set within the medical home (such as a flu vaccination) is automatically carried over to the social network.  In the event that the patient is due or overdue according to that date, they will see an alert about it without ever having to enter that date within the social network application itself.

• Activity Tracking - the social network application will also update the medical home with how frequently the patient uses the social network and what they do within the site.  These statistics will be available when
  a patient-approved provider accesses that patient's information.  Aggregate data will be accumulated over time to measure the effectiveness of the application.

• Reference Library - contains various articles and blurbs that will attempt to further motivate the patient to take better care of their health.  It also includes recipes that the patient can follow to help manage their chronic disease through better nutrition.  System coordinators will have the ability to update these libraries with new information and articles as needed. 

• Text Messaging - system coordinators will be able to send motivational messages to patients via SMS text messaging on a daily and weekly basis.  Patients have the ability to choose whether or not they wish to receive these text messages. 

• Fan Club - the patient will be allowed to invite their friends and family to become a part of their “fan club.”  These fan club members can then issue challenges to the patient, and even participate in challenge with them.  They can also see the progress that the patient is making in a variety of different statistical areas.

• Full Patient Control - the patient has complete control over the information that they entered within the social network, and what gets shared with their fans or their providers.  The social network has several built-in toggles that the patient can set to share any information as desired, including privacy settings, text messaging options, and fan club access settings.
